Liverpool University Hospitals NHS Foundation Trust takes the health and well-being of it’s staff very seriously.

Within the Occupational Health and Wellbeing Service our aim is to help you improve your health so that you stay fitter longer.

Healthy staff provide better patient care as well as having a more fulfilling work life balance.

 

Liverpool University Hospitals have now launched an online Health Assessment form.

Recruitment will send a link to the new employee who will be able to complete their Health Questionnaire online and upload any relevant documents such as immunisation history from other employers.

Occupational Health will endeavour to health clear the new employee with this information and may contact them if an appointment is needed.

A Health Clearance note will be sent to Recruitment to show Occupational Health clearance.
