Access to Health Records

Under the General Data Protection Regulation you have the right to request from us a copy of your health records. This is known as a Subject Access Request.

Proof of Identity

If you are applying for access to your records, you will need to send proof of identity. Please send a copy of your passport, photo driving license or equivalent identification

Fees for Access to Health Records

There is no charge for processing these requests. However, if a request is excessive or repetitive, a reasonable fee may be charged, you will be advised of this once your request has been assessed.

 

 

How long is the process

When the written application has been received containing all the information needed to process the request, then this must be completed within one month. If the requests is complex or numerous, then this period can be extended by a further two months. If this is the case,then we will contact you to explain why the extension is necessary.

 

The records will be posted to you via 2nd class post. Please advise us if you require your health records to be provided in an alternative format.
